Looks like on the right side of her face is only little make-up. I mean you could easily cover the shadow under the eye with some cooling eye cream, a good concealer and a little powder.

If it’s real, she still looks great.
However, I think the person who did this also did some photoshoppin gto the right side as well.
They added little hairs, but went to far and all the way up past her right eye (her right, not yours) it looks like smudge.
Madonna looks great on either side.
She has oxygen facials, expensive creams, exercise, & macrobiotic diet & some “work” to look the way she does which is great. If only she tried raw foods.
